Background
ICH5244 is a research-grade biosimilar of ginisortamab, supplied as an unconjugated, non-therapeutic recombinant analog of the originator anti-Gremlin-1 (GREM1) antibody. It is a human IgG4-kappa built around the human GREM1 target (UniProt O60565) and is intended strictly for research use, not for human or veterinary application. The molecule is useful as a functional reference standard for investigators studying GREM1-directed antagonism: it supports binding and neutralisation assays, serves as a positive/isotype-matched control in BMP-pathway experiments, and provides a consistent tool for in-vitro and preclinical characterisation of GREM1 biology. Because it is manufactured as a research reagent, it is offered at low endotoxin (research grade below 1 EU/mg; ultra-low below 0.5 EU/mg) and in bulk milligram-to-gram quantities suitable for assay development, screening cascades, and comparability or bridging work. As a biosimilar analog it recapitulates target recognition without being formulated or released as the clinical drug, making it appropriate for benchwork where reproducible, well-defined GREM1 engagement is required rather than therapeutic administration. Gremlin-1 (GREM1) is a secreted, cysteine-knot glycoprotein of the DAN family and a potent extracellular antagonist of bone morphogenetic proteins (BMPs), notably BMP-2, BMP-4 and BMP-7. By binding these ligands in the extracellular space, GREM1 prevents their engagement of BMP type I/II receptors, thereby restraining SMAD1/5/8 signalling and tuning the balance between BMP and TGF-beta pathway activity. GREM1 is essential in embryonic patterning, limb and kidney development, and it also modulates adult tissue homeostasis. Independently of BMP sequestration, GREM1 can act as an agonist ligand for VEGFR2, promoting angiogenic signalling. Aberrant GREM1 expression is implicated in fibrosis, and in the tumour microenvironment and cancer-associated fibroblasts, where elevated GREM1 supports stromal remodelling, proliferation and disease progression, motivating therapeutic neutralisation.
